Accidents, Incidents & Issues

ACCIDENT reports must be competed by the leader or person in charge at the time of the accident. A report must be competed for all accidents, regardless of severity. An insurance form must be completed by the leader and a physician, and submitted to council for endorsement. Council will provide this form.

INCIDENT reports are used to document circumstances, actions, or situations that are beyond normal conditions or normal behavior. For example: arguments/fights, emotional outbursts, lost child, "near miss" accidents, radically threatening weather, witnessing a traumatic event, etc.

 

When in doubt, FILL IT OUT!

 

Reports should be submitted directly to GSCSNJ, and cc'd to Roxanne & Heidi.
